Hendersonville, NC  Teleios Collaborative Network (TCN) announced the addition of Hospice of Eastern Idaho to its network. Hospice of Eastern Idaho is Southeast Idaho’s only nonprofit hospice organization.

“We are excited to welcome Hospice of Eastern Idaho to our Network. They are notably the only nonprofit hospice organization in all of Southeast Idaho. We recognize that in today’s changing healthcare landscape, with ever increasing regulatory requirements, that it is challenging to provide true patient-focused care and to have the resources that are needed to expand services. By providing resources, leadership, and expertise shared across the network, Teleios enables its members to focus on what matters most, quality patient care and increased accessibility,” said Chris Comeaux, President and CEO of TCN.

“Hospice of Eastern Idaho is very excited to join the Teleios Collaborative Network. Collaborating with and learning from committed and likeminded nonprofit hospice agencies across the country will only enhance the already excellent services we provide to our neighbors in Eastern Idaho,” said Celeste Eld, Executive Director of Hospice of Eastern Idaho.

With the addition of Hospice of Eastern Idaho, Teleios has over 3,350 hospice and 4,150 palliative care patients in its network.

TCN is a nonprofit organization that has created a clinically integrated network that shares expert leadership, industry best practices, and resources with its member organizations, allowing community-based, nonprofit Hospice and Serious Illness agencies to continue their work of providing compassionate care for those facing serious illness or the end of life. TCN was founded in 2017 by Four Seasons and Carolina Caring and co-founded by AMOREM and Mountain Valley Hospice and Palliative Care organizations. TCN is currently comprised of twelve member organizations and serves in:

  • Idaho
  • Louisiana
  • North Carolina
  • South Carolina
  • Texas
  • Virginia
