Teleios Announces Community Nursing Services (CNS) Joins Network

Hendersonville, NC Teleios Collaborative Network (TCN) announced the addition of Community Nursing Services (CNS) to its Network. CNS is the oldest nonprofit home health and hospice provider in Utah and serves most of the state.
“CNS is a premier provider of home health and hospice services. They have a great reputation in providing quality services to patients and families over many decades. We are honored to welcome them into the Network. In an environment where nonprofit hospices are selling to for profits at a rapid pace, we recognize that the Network offers support to those nonprofits that remain so that they can focus on what they do best. The healthcare landscape has ever increasing regulatory requirements and it is challenging to provide true patient-focused care and have the resources that are needed to expand services. By providing support, leadership, and coaching expertise in a number of areas shared across the network, Teleios enables its members to focus on what matters most, quality patient care and increased accessibility,” said Chris Comeaux, President and CEO of TCN.
Brent Jones, President of CNS said, “After almost 95 years of service to the Utah community, CNS strives to continually evolve and stay relevant. Our team is anxious to work with the experts at TCN to continue on our journey of growth.”

With the addition of CNS, Teleios has over 3,500 hospice and 4,300 palliative care patients in its network.


TCN is a nonprofit organization that has created a clinically integrated network that shares expert leadership, industry best practices, and resources with its member organizations, allowing community-based, nonprofit Hospice and Serious Illness agencies to continue their work of providing compassionate care for those facing serious illness or the end of life. TCN was founded in 2017 by Four Seasons and Carolina Caring and co-founded by AMOREM and Mountain Valley Hospice and Palliative Care organizations. TCN is currently comprised of twelve member organizations and serves in:

  • Louisiana
  • North Carolina
  • Texas
  • Utah
  • Virginia
