Teleios Announces Addition of Central Wyoming Hospice & Transitions

Hendersonville, NC April 3, 2025:  Teleios Collaborative Network (TCN) announced the addition of Central Wyoming Hospice & Transitions as a member. Since 1981, Central Wyoming has been delivering compassionate hospice services, ensuring comfort, dignity, and peace for patients and their families.  2005 Transitions was established to help community members remain independent by offering personalized support and connecting them to essential resources. In addition to hospice and transitions, Central Wyoming also provides grief support to the community and offers the We Honor Veterans Program.

Kilty Brown, Executive Director of Central Wyoming Hospice & Transitions, said of joining, “We’re thrilled to join TCN and be part of a network of nonprofit hospices who share our heart for innovation and truly excellent care.  This is an important step for us as we grow and deepen the support we offer to individuals and families across Wyoming.”

Teleios Collaborative Network is a nonprofit organization that has created a clinically integrated network that shares expert leadership, industry best practices, and resources with its member organizations. The goal of TCN’s collaboration is to harness the best of each hospice and enable the network to care for the patients and families in each community served. There are over 3,500 hospice patients and over 4,300 palliative patients in its network. TCN was founded in 2017 by Four Seasons and Carolina Caring and co-founded by AMOREM and Mountain Valley organizations. TCN is currently comprised of seventeen member organizations across eleven states.
